Is a standard website contact form HIPAA compliant for a medical practice?
Usually not. A name plus a condition or a treatment request is protected health information, so the form tool, the email that delivers it, and the CRM that stores it each need a business associate agreement. Most form builders will not sign one.
The fix is a form that posts to a BAA covered backend with only a link in the notification email, consent text on the form, and no third party pixels on the page. That is our HIPAA form and call stack add on at $750 a month.
